Internship Application
This form is intended to meet application needs for the internship program. It asks a series of questions related to experience, skills, available time, education, and interests. It requires potential interns to upload a resume.
Demographic Questionaire
This form is submitted anonymously and consists of a series of questions specific to a users background, ethnicity, age, education, etc… If used properly, it should allow Deproduction to more effectively target and market its class offering as well as provide needed information to existing and potential funders. It is currently linked to the Class Registration form, but could also be used in other settings such as User signup.
Contact Concerning Demographic Questionaire
This is a simple form that can be used as a model for any ‘contact us’ pages or needs. It consists of a simple description, a flexible textfield, and a user defined email field.
Volunteer Registration Form
This form is intended to meet registration needs for the volunteer department. It asks a series of questions related to experience, skills, available time, and interests. It allows potential volunteers to upload a resume or other documentation if they so choose.
DOM Class Registration
This form allows users to register for any studio, field, or editing class offered through DOM. It should be updated to included new class dates and times. It also allows users to select payment options and links users to specified payment destinations.

This form allows volunteers to give feedback on their experience with DOM and consists of a series of questions mostly answerable on a scale of 1-5. It should allow DOM to asses its volunteer programs and improve where necessary. This form is submitted anonymously.
Certification Course Feedback
This form allows class takers to provide feedback on the utility and effectiveness of DOM’s certification classes. Most questions are on a scale of 1-5. This form is submitted anonymously.
